ABOUT THE ROLE

The Nurse Practitioner (NP) will work independently as well as with the and is responsible for providing physical and mental health assessments and treatment for vulnerable clients that are not connected to any other services in the community and team reports to the Coordinator - GAPPS (Getting Appropriate Personal and Professional Supports) Program. Works collaboratively within the GAPPS tri-partner agency team.

WHAT YOU BRING

Education/Experience:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice
  • Several years of experience in community health and/or mental health
  • Membership in professional association e.g. NPAO

 

Skills/Abilities:

  • Advanced clinical experience skills and judgement in mental health
  • Superior ability to communicate effectively and concisely both orally and in writing and through a variety of means
  • Strong knowledge of community and regional services
  • Ability to function and contribute as an effective team member including providing mentoring education and coaching within the team
  • Ability to work independently with the capacity to case review and consult with physicians
  • Strong leadership skills and demonstrated ability to organize prioritize and multitask efficiently
  • Excellent computer presentation and educational skills
  • Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability
  • Effective listening observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
  • Ability to use independent judgment and maintain confidentiality of records actions and information

 

Conditions of Employment:

  • Maintain certifications
  • Ensure appropriate Professional Liability Protection is available according to the requirements of the CNO
  • Valid Ontario Driver's Licence: Class G
  • This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
  • Offers of employment made to external candidates are conditional upon providing proof of a complete primary series of Health Canada approved mRNA COVID-19 vaccine or proof of valid medical exemption, prior to the offered effective date. If proof of vaccination or valid medical exemption is not provided, the offer of employment is rescinded.
